Patient Code of Conduct
Welcome to Lavender Spectrum Health. We are committed to providing you with high-quality care in a supportive and respectful environment. To ensure the best possible experience for all our patients, staff, and providers, we ask that you adhere to the following Code of Conduct:
Respect and Courtesy
Treat all staff, providers, and fellow patients with respect and courtesy.
Refrain from using offensive, discriminatory, or abusive language and behavior.
Privacy and Confidentiality
Respect the privacy of other patients by maintaining confidentiality and not inquiring about other patients' conditions or treatments.
Protect your own privacy by discussing your medical information only with authorized personnel.
Appointments
Arrive on time for your appointments, defined as within 10 minutes of the start time of an appointment. If you are unable to attend, please notify us at least 24 hours in advance to reschedule or cancel.
Be prepared for your appointment by bringing necessary documents, a list of current medications, and a list of your concerns to address during the appointment.
Communication
Communicate openly and honestly with your healthcare provider about your health concerns and medical history.
Follow the recommended treatment plans and ask questions if you do not understand your care instructions.
If you do not agree with your treatment plan or have concerns, you have the right to voice your concerns and ask for alternatives
If safe alternatives are not available, your provider will explain the risks and benefits of not following the recommended treatments
Your provider will work with you to give you as much choice and autonomy as is possible in the context of your specific health concerns and work to incorporate your values and preferences whenever it is safe to do so
If voicing your concerns, requesting alternatives, requesting clarification, or providing your own input is interfering with your provider’s ability to provide safe and effective care to yourself or others, this may constitute disruptive behavior and you may be asked to refrain from these behaviors or schedule additional appointments
If your provider recommends a treatment plan, and there are no safe alternatives, failure to follow through with the treatment plan may result in discharge if failing to follow through with the treatment plan has a high likelihood of harm to yourself or others. Your provider will explain the reasoning for recommending a certain treatment plan and why alternatives may not be safe.
Please be mindful the maximum amount of face-to-face time with a primary care provider to be 45 minutes in a new patient appointment and 20 minutes in a follow up appointment and we need your cooperation to be able to run on time to respect the time of all our patients and staff.
Safety and Cleanliness
Follow all posted safety guidelines and instructions from staff.
Maintain cleanliness in all areas of the practice, including waiting areas and restrooms.
Financial Responsibility
Understand your insurance coverage and be prepared to fulfill any financial obligations, including co-pays and deductibles, at the time of your visit.
Contact our billing department if you have questions or concerns about your bill.
Zero Tolerance Policy
We have a zero-tolerance policy for violence, threats, or any form of harassment. Such behavior may result in termination of your care at our practice.

We welcome your feedback. If you have any concerns or suggestions about your care or our services, please speak with a staff member or contact our office manager.
